Regional plans for health priorities
We engage with a broad range of stakeholders to ensure planning and commissioning approaches reflect the specific needs and issues within each health priority. See plans below that address some of these health priorities.
Building on this approach and successes to-date, the PHN will adopt a similar regional planning process to respond to the other health priorities.
Five Year Health Care Plan For Older People Who Live In Brisbane North | 2017-2022
Brisbane North PHN and Metro North Hospital and Health Service (MNHHS) have developed a five year health care plan for older people who live in Brisbane North which will help to address the health priority of coordinated care for older people until 2022.
This plan will guide the way healthcare services are delivered to this group of people in our community and represents a commitment between the PHN and MNHHS to improve the coordination, integration and provision of care services to support the health and wellbeing of older people to help them stay healthy and at home.
Planning For Wellbeing | 2018-2023
A Regional Plan for North Brisbane and Moreton Bay focusing on mental health, suicide prevention and alcohol and other drug services 2018-2023.
Brisbane North PHN and Metro North Hospital and Health Service (MNHHS) have sponsored the development of a plan to improve mental health, suicide prevention and alcohol and other drug treatment services in North Brisbane and Moreton Bay.
The regional plan sets out the challenges facing us and what we can all do over the next few years to improve the quality, coordination and integration of services.
Most importantly it is a plan about the type of community we want to build and it’s about the lives of those affected by mental illness, suicide or problematic drug use.
