Evaluation of the PHN’s Team Care Coordination program

An evaluation of Brisbane North PHN’s Team Care Coordination program has confirmed it is effective at reducing Emergency Department (ED) attendances, hospital admissions and the severity of patient illness. 

The Centre for Health Economics Research and Evaluation at the University of Technology Sydney was engaged to undertake the evaluation and delivered its report in October 2017.

PHN involvement in the wounds management innovation cooperative research centre

Brisbane North PHN is part of the Chronic Wounds Solutions Collaborating Group and has supported the development of a recommendations paper.

As a member of the Group, Brisbane North PHN participated in the report’s launch on 15 March 2018 at Metro North Hospital and Health Service’s 2018 Chronic Wound Care Conference.

Steady, Invested, Strong – A Consortium and Commissioning Toolkit

As part of our integrated and collaborative approach to commissioning, Brisbane North PHN has developed a toolkit that tells the story of work undertaken by the PHN, in conjunction with our partners, through three consortia.

Freely available for download and use, the toolkit tells the story of these consortia through a practical guide rather than a theoretical discussion of collaboration and integration.
