EOI for My Health for Life providers now open
Brisbane North PHN is currently promoting expressions of interest for providers to deliver the My Health for life evidence-based lifestyle modification program in the North Brisbane and Moreton Bay region.
My health for life is a Queensland integrated risk assessment and behaviour modification program that aims to assist participants to reduce their risk of developing chronic conditions such as type 2 diabetes, heart disease and stroke.
Funded by the Queensland Government and delivered by the Healthier Queensland Alliance, the program encourages and supports participants to achieve and maintain healthy lifestyle goals.
Based in the community and delivered by local healthcare practitioners, this initiative will identify those at high risk of developing chronic conditions and offer them a tailored, but flexible, healthy lifestyle program.
Interested provider organisations are requested to complete the Expression of Interest and return to firstname.lastname@example.org by Friday 6 July 2018. Please note this is not a tender process, multiple organisations will be approved to deliver the program from the same region.
Once all EOIs are received they will be assessed by the My health for life Operations Management Committee. All applicants will be provided with a response to their EOI via return email by Wednesday 18 July 2018.
Successful organisations will then need to identify appropriately qualified staff to attend facilitator training on 30 - 31 August 2018 (location to be confirmed).
