Dementia Project Steering Committee EOI now open

The Brisbane North PHN and Metro North Hospital & Health Service have worked together to produce a Five-year health care plan for older people who live in Brisbane North 2017-2022. The direction outlined in the plan is focused on older people who are vulnerable and experiencing cognitive impairment including dementia and delirium.

In understanding the importance of supporting people with this growing issue, the Brisbane North PHN has initiated a dementia project. The project aims to enhance dementia care for people living with dementia, their families and carers in the the Redcliffe area.

Due the project’s focus on the Redcliffe area, we are seeking a consumer representative who is living in Redcliffe to join our Dementia Project Steering Committee (PSC). As a member, you will be instrumental in providing your personal experience as a person living with dementia or a carer for a person living with dementia, to contribute to the development and implementation of the project.

The Steering Committee will meet on a monthly basis for the first three months and bi-monthly subsequently for 90 minutes alternate between Brisbane North PHN offices in Lutwyche shopping centre and North Lakes. A sitting fee and reimbursement for travel is provided. Other supports are also provided (e.g. induction, training, disability support).

To apply please complete the expression of interest form and email the completed form to amy.petrocy@brisbanenorthphn.org.au. Expressions of interest close at 5.00pm on Wednesday 31 October 2018.

For further information about the project please contact Mai Eames, Development Lead, Aged and Community Care, Brisbane North PHN on (07) 3630 7368 or via email mai.eames@brisbanenorthphn.org.au.
