Staying Healthy, Staying Home
Team Care Coordination is now accepting referrals from Metro North Hospital and Health Service Facilities as part of the Staying Healthy, Staying Home program.
Patients who do not require urgent complex clinical service but who may benefit from support at home may be eligible for support under Team Care Coordination.
Team Care Coordination provides a coordination role for patients who have chronic complex health conditions.
Our Team Care Coordinators are all Registered Nurses, experienced in the community and in general practice, who have a wealth of knowledge about local public, community and private healthcare services.
They have the capacity to visit people either in their homes or at the practice to conduct a comprehensive assessment, and to liaise with not just the GP and patient, but hospitals and other health and social services the patient may need.
Hospital referral to Team Care Coordination
If you work in a Metro North Hospital and Health Service facility, you can use the Queensland Health Team Care Coordination Referral form (Word, 84KB) to refer eligible patients to Team Care Coordination.
