Services for people with severe mental illness
Brisbane North PHN supports integrated clinical and nonclinical services for people with severe mental illness through three mental health service hubs.
The hubs deliver services across the region with their locations aligned to the Metro North Mental Health Service catchment areas. Each hub is operated by a different provider with extensive experience in providing services to people with severe and complex mental illness.
In the Royal Brisbane and Women’s Hospital catchment: Communify, in partnership with Toowong Private Hospital, Brisbane Youth Service, Open Minds, Eating Disorders Queensland and Wesley Mission operate The Recovery and Discovery Centre.
In The Prince Charles Hospital catchment: Neami National operates The Living and Learning Centre.
In the Redcliffe and Caboolture Hospital catchment: Aftercare operates Floresco Caboolture.
Introduction to the service
Each mental health hub provides integrated services including:
- assessment, triage and intake
- service navigation
- care coordination
- psychological therapy (mostly group)
- physical health care
- psychosocial support
- assistance to access the NDIS
- a range of in reach services provided by external providers (e.g. AOD treatment, employment, housing, finance)
- each hub also provides outreach services.
To be eligible people must:
- be 18 years and over
- have a diagnosable severe or severe and complex mental illness (episodic or persistent)
- be experiencing significant disruption to their daily life, wellbeing and functioning.
Please note that people who are experiencing acute mental illness should continue to be referred to Metro North Mental Health Services via 1300 MH CALL.
The mental health hubs are commissioned by Brisbane North PHN and there is no cost to the client.
The Recovery and Discovery Centre operates Monday to Friday 9am to 5pm with on call support outside of these hours available. The centre also provides some weekend activities.
The Living and Learning Centre operates Monday to Friday 8am to 8pm.
Floresco Caboolture operates Monday to Friday between 9am and 5pm.
Anyone can refer to the mental health hubs with the consent of the person being referred.
There are no referral boundaries and people can access whichever mental health hub is most convenient to them.
GP’s can refer by completing the My Mental Health Services eReferral form via GP Integrator or HealthPathways. Please note that GP Integrator is the preferred referral method for GPs as it includes various benefits including prepopulation of your details and feedback on referrals. If your organisation does not have this software installed we recommend you contact your Brisbane North PHN Primary Care Liaison Officer (PCLO) to organise installation.
Non GP service providers can refer by completing referral form on HealthPathways.
