Our approach to stepped care
Connecting people to the right services, at the right time and in the right place
Following consultation with consumers, carers and service providers, Brisbane North PHN has released an updated video and factsheet explaining our vision for stepped care.
We are committed to ensuring people receive the right services at the right time and in the right place. Using our primary mental health care funds we will purchase services within a stepped care approach.
Through our regional planning role we will promote a stepped care approach across the mental health system.

Initial Assessment & Referral Guidance for Mental Health – Implementation Trial
Brisbane North PHN is participating in a trial of a new Department of Health initiative. The Initial Assessment and Referral (IAR) guidance has been designed to aid referrers in determining the best level of care for a client. The guidance features eight domains that assist in rating an individual’s presentation and a decision support logic that will calculate the appropriate level of care required within a stepped care approach.
