Health Care Home
Following an application and selection process, the Department of Health has announced the preferred General practices and Aboriginal Community Controlled Health Services (ACCHS) in the Brisbane North PHN catchment area to become a Health Care Home.
200 practices have been shortlisted nationally with twenty practices to commence delivering services as a Health Care Home on 1 October 2017. The remaining 180 practices will begin on 1 December 2017.
The list of 200 selected practices is available at the Tenders and Grants page, successful grant recipients, on the Department of Health website. The next step is for those participating practices to complete contract negotiations with the Department of Health before commencing the process of becoming a Health Care Home.
