Team Care Coordination
Team Care Coordination is an independent program providing GPs and their patients with the expert services of our Team Care Coordinators.
Team Care Coordinators are Registered Nurses, experienced in the community and in general practice, who have a wealth of knowledge about local public, community and private healthcare services.
Our Coordinators can assist GPs to assess a person’s health and care needs and to arrange the services they need to stay healthy and active.
Our team has the capacity to assist with care planning, to visit people either in their homes or at the practice, and to liaise with GPs, patients, hospitals and other health services. Team Care Coordinators can contribute to GP Management Plans and can be a member of the care plan team.
Team Care Coordination is managed by the Brisbane North PHN and supported financially by Queensland Health.
An evaluation of Brisbane North PHN’s Team Care Coordination program has confirmed it is effective at reducing Emergency Department (ED) attendances, hospital admissions and the severity of patient illness. The Centre for Health Economics Research and Evaluation at the University of Technology Sydney was engaged to undertake the evaluation and delivered its report in October 2017.
