The Health Navigator is a free online tool designed to help people identify their risk for developing chronic diseases such as heart attack, stroke, kidney disease and diabetes.
The Health Navigator also has links to a range of lifestyle programs and other resources to assist health professionals in referring patients into local programs.
The Health Navigator assessment was developed in collaboration with the George Institute for Global Health, University of Sydney and is based on recommendations from several Australian cardiovascular disease, kidney disease, diabetes and preventive health guidelines.
The Health Navigator uses smart phone and mobile web technology to raise awareness of chronic disease risk factors, improve health literacy, and navigate the user to appropriate support services.
The development of the Health Navigator mobile website is a platform from which the community can gain awareness of lifestyle risk factors, access health professionals and self-refer to free or low-cost local lifestyle modification programs and other local healthy eating, active lifestyle opportunities.
Through the use of modern technology and by keeping up with how people connect and communicate, we aim to maximise the reach of our health promotion campaigns and offer direct access to disease prevention knowledge, support and activities.
